Indications/Uses
Peripheral & central neuropathic pain, generalized anxiety disorder (GAD). Adjunctive therapy in partial seizures w/ or w/o secondary generalization. Management of fibromyalgia.
Dosage/Direction for Use
Adult 150-600 mg/day in 2 or 3 divided doses. Neuropathic pain Initially, 150 mg/day in 2 or 3 divided doses. May be increased to 300 mg/day after 3-7 days. Max: 600 mg/day after an additional 7-day interval. Epilepsy Initially, 150 mg/day in 2 or 3 divided doses. May be increased to 300 mg/day after 1 wk. Max: 600 mg/day after an additional wk. GAD 150-600 mg/day given in 2-3 divided doses. Initially 150 mg/day, may be increased to 300 mg/day after 1 wk. Following an additional wk, dose may be increased to 450 mg/day. Max: 600 mg/day after an additional wk. Fibromyalgia 300-450 mg daily. Initially 150 mg daily. May be increased to 300 mg daily w/in 1 wk. Max: 450 mg daily. Renal impairment CrCl ≥60 mL/min Initially 150 mg/day to a max dose of 600 mg/day bid or tid, ≥30 to <60 mL/min Initially 75 mg/day to a max dose of 300 mg/day bid or tid, ≥15 to <30 mL/min Initially 25-50 mg/day to a max dose of 150 mg/day once daily or bid, <15 mL/min 25 mg/day to a max dose of 75 mg/day once daily.
Administration
May be taken with or without food.
Special Precautions
Diabetes. Discontinue if symptoms of angioedema occur. Dizziness, somnolence, loss of consciousness, confusion, mental impairment, blurred vision, CHF. Renal failure. W/drawal of concomitant antiepileptic drugs. Monitor for signs & symptoms of suicidal ideation & behaviour. Concomitant use w/ anti-spasticity agents. Consider prevention of constipation (especially female & elderly patients) & reduced lower GIT function may occur in concomitant use w/ opioid analgesics. History of drug abuse. Encephalopathy. May impair ability to drive or operate machinery. Pregnancy & lactation. Adolescents (12-17 yr) & childn <12 yr. Elderly.
Adverse Reactions
Dizziness, somnolence. Increased appetite; euphoric mood, confusion, irritability, decreased libido, disorientation, insomnia; ataxia, abnormal coordination, tremor, dysarthria, memory impairment, attention disturbance, paraesthesia, sedation, balance disorder, lethargy, headache; blurred vision, diplopia; vertigo; vomiting, dry mouth, constipation, flatulence; erectile dysfunction; abnormal gait, feeling drunk, fatigue, peripheral edema, edema; increased wt.
Drug Interactions
May potentiate effects of ethanol & lorazepam. Resp failure & coma may occur w/ other CNS depressants. Additive effect in the impairment of cognitive & gross motor function caused by oxycodone.
